The family portal is designed to allow you to interact with our office and caregivers in real time. You will have access to your loved one’s schedule, the name of the caregiver scheduled, plan of care, visit record, and much more. This allows true transparency and instant access to information. You can allow as many family members as you wish to access the portal.
To access the family portal, you will need a Username, Password and Account ID number. Please contact our office directly to obtain your Client ID number and Account ID number.
